Blaise Health

1. Who We Are and What This Policy Covers
This Privacy Policy describes how Blaise Health ("we," "us," or "our"), collects, uses, shares, and protects personal information when you visit www.BlaiseHealth.com (the "Site"), use our contact form, or contact us by phone, email, or text message in response to the Site.
This Policy covers the Site and your inquiries to us. It does not cover treatment records. If you become a patient, the health information we create or receive in providing care is protected by HIPAA, 42 C.F.R. Part 2 (federal confidentiality rules for substance use disorder treatment records), the California Confidentiality of Medical Information Act, and other laws, and is handled as described in our Notice of Privacy Practices. If this Policy and the Notice of Privacy Practices conflict about patient health information, the Notice of Privacy Practices controls.
By using the Site, you acknowledge this Policy. If you do not agree, please do not use the Site.
2. Please Do Not Send Sensitive Health Information Through the Site
Our contact form, email, and text messages are not guaranteed to be secure. Please do not include diagnoses, medications, substance-use or treatment history, Social Security numbers, insurance member IDs, or payment card information in the form or in emails and texts. Keep your message general. We will collect what we need securely when you speak with our admissions team or are admitted.
3. Information We Collect
a. Information you give us. When you submit our contact form we collect your first and last name, email address, phone number, type of inquiry (selected from a drop-down menu), and the message you write. If you call, email, or text us, we collect the information you share with us, such as your name, phone number, and what you tell us about your inquiry.
b. Information collected automatically. When you visit the Site, our systems and service providers automatically collect technical information, which may include your IP address, browser type and settings, device type, operating system, language, pages you view, links you click, the date and time of your visit, the website that referred you, and an approximate location (such as city or region) derived from your IP address. We collect this using cookies and similar technologies (see Section 6).
c. What we do not intentionally collect through the Site. We do not use the Site to intentionally collect Social Security numbers, payment card numbers, precise geolocation, biometric information, or insurance information.
d. Information from other sources. We do not buy personal information about you from data brokers or lead-generation companies.
4. How We Use Your Information
We use the information described above to:
respond to your inquiry and provide information about our services and admission process;
contact you by phone, text, or email about your inquiry, as described in Section 7;
operate, secure, maintain, and improve the Site, including by understanding how visitors use it;
detect, prevent, and investigate fraud, abuse, security incidents, and violations of our Terms of Use;
comply with legal obligations, respond to lawful requests, and establish or defend legal claims; and
for any other purpose described to you at the time you provide the information, or with your consent.
We do not use contact-form information to build advertising profiles, and we do not use it to target ads to you on other websites.
5. How We Share Your Information
We do not sell your personal information, and we do not share it with third parties for cross-context behavioral advertising. We disclose personal information only as follows:
Service providers. Companies that help us run the Site and respond to inquiries under written contracts that limit their use of the information, such as website hosting, email and phone systems, form or customer-relationship-management software, security tools, and website analytics. Where required for health information, we use appropriate agreements, such as business associate agreements.
Legal and safety. When we believe disclosure is required by law, subpoena, or court order, or is necessary to protect the rights, property, or safety of you, us, or others, including in an emergency.
Business transfers. In connection with a merger, acquisition, financing, or sale of all or part of our business, subject to appropriate confidentiality protections.
With your consent or at your direction.
If you are admitted as a patient, disclosures of your treatment information are governed by your signed consents
and the Notice of Privacy Practices, not by this Section.
6. Cookies, Analytics, and Do-Not-Track
We and our service providers use cookies and similar technologies to keep the Site working, remember preferences, keep it secure, and measure traffic. We use built-in website analytics to understand how the Site is used. We do not use advertising pixels, remarketing tags, or session-replay tools on the Site.
Your choices. You can manage cookies through the cookie banner on the Site and your browser settings. Blocking some cookies may affect how the Site works.
Global Privacy Control and Do-Not-Track. We do not sell or share your personal information for advertising, so there is nothing for an opt-out signal to opt you out of. Because there is no uniform standard for Do-Not-Track browser signals, we do not respond to them.
7. Calls, Texts, and Email
If you give us your phone number, we may call or text you about your inquiry and admission, as described in our Terms of Use. Message and data rates may apply, and message frequency varies. Reply STOP to any text to opt out, or tell us during a call. Consent to texts is not required to receive services. We may still send non-marketing messages about an inquiry you started. Calls with our team may be recorded or monitored for quality, training, and compliance, and we will tell you at the start of the call. You can unsubscribe from any marketing email by using the link in it. Do not use texts or email to share sensitive health information (see Section 2).
8. How Long We Keep Information
We keep contact-form and inquiry information for as long as needed to respond to you and for our legitimate business, legal, and compliance purposes, which is generally 24 months after your last contact with us, unless a longer period is required or permitted by law or you become a patient (in which case treatment records are retained under applicable law and our records policies). Website analytics data is kept for 14 months. We then delete or de-identify the information. Backup copies may persist for a limited time before they are overwritten.
9. How We Protect Information
We use reasonable administrative, technical, and physical safeguards designed to protect personal information, including encryption in transit (HTTPS), access controls, and vendor oversight. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. Please use the Site and our contact form at your own risk and follow Section 2. If a breach affects your information, we will notify you and regulators as required by law.
10. Your California Privacy Rights
This Section applies to California residents to the extent the California Consumer Privacy Act, as amended by the California Privacy Rights Act (the "CCPA"), applies to us. The CCPA does not apply to protected health information governed by HIPAA, to substance use disorder records governed by 42 C.F.R. Part 2, or to medical information governed by the California Confidentiality of Medical Information Act; those are governed by the Notice of Privacy Practices.

California "Shine the Light" (Civil Code § 1798.83). We do not disclose personal information to third parties for their direct marketing purposes. California residents may request confirmation of this at the contact details below.

You may exercise any such right using the contact details in Section 10, and we will honor requests as required by the law that applies to you.
12. Children's Privacy
The Site is not directed to children under 13, and we do not knowingly collect personal information from anyone under 18 through the Site without a parent or guardian's involvement. If you are a parent or guardian and believe a minor has sent us personal information, contact us and we will delete it. Treatment of minors, where offered, is governed by separate consents and applicable law.
13. Third-Party Websites and Services
The Site may link to other websites, such as insurers, maps, social media, or resources. We do not control and are not responsible for their privacy practices. Please review their policies before sharing information.
14. International Visitors
The Site is operated in and intended for use in the United States. If you access it from elsewhere, you understand your information will be transferred to and processed in the United States.
15. Changes to This Policy
We may update this Policy from time to time. We will post the updated version with a new "Last updated" date, and, if the changes are material, we will provide additional notice as required by law. Your continued use of the Site after an update means you accept the updated Policy.
