Terms of Use

Blaise Health

Last updated: October 13, 2026

1. Agreement to These Terms

These Terms of Use (the "Terms") are a binding agreement between you ("you") and Blaise Health, a California corporation ("Company," "we," "us," or "our"). They govern your access to and use of www.BlaiseHealth.com and any related pages, our contact form, and other online services that link to these Terms (together, the "Site").

By accessing or using the Site, clicking "I agree," or submitting a form, you accept these Terms and our Privacy Policy and Notice of Privacy Practices. If you do not agree, do not use the Site.

These Terms apply only to the Site. They do not govern in-person or telehealth clinical treatment, which is governed by separate written admission, consent, and financial agreements (see Section 3). If a separate signed agreement conflicts with these Terms regarding clinical services, the signed agreement controls for those services.

2. No Medical Advice; No Provider-Patient Relationship; Emergencies

The Site is for general information only. Nothing on the Site, including text, graphics, service descriptions, or statements by non-clinical staff, is medical, psychiatric, psychological, legal, or other professional advice, diagnosis, or treatment, and none of it should be relied on as such.

Using the Site, contacting us, submitting the contact form, or speaking with an admissions representative does not create a provider-patient or therapist-client relationship. That relationship arises only after you are formally assessed and admitted under a signed admission agreement.

Do not ignore professional advice or delay seeking care because of something you read on the Site.

IF YOU ARE EXPERIENCING A MEDICAL OR PSYCHIATRIC EMERGENCY, INCLUDING OVERDOSE, WITHDRAWAL COMPLICATIONS, OR THOUGHTS OF SUICIDE OR SELF-HARM, CALL 911 IMMEDIATELY OR GO TO THE NEAREST EMERGENCY ROOM. In the United States you may also call or text 988 (Suicide & Crisis Lifeline). The Site and contact form are not monitored for emergencies and must not be used to seek emergency help.

3. Treatment, Admission, and Insurance Information

(a) Admission is not guaranteed. All admissions depend on clinical assessment, medical necessity, level-of-care criteria, availability, and our admission policies. We may decline or discharge any person in accordance with law and our policies.

(b) No guarantee of outcomes. Recovery outcomes vary by individual. We do not promise or guarantee any result, including abstinence, relapse prevention, symptom improvement, or length of recovery. Statistics, success descriptions, and testimonials on the Site are general, may not be typical, and are not predictions of your results.

(c) Insurance and costs. Any cost estimate, insurance statement, or coverage statement given through the Site or by staff is an informational estimate only and is not a guarantee of coverage, authorization, or payment. Your insurer makes final coverage determinations. You remain responsible for charges as set out in your financial agreement. Rates, programs, and amenities may change without notice.

4. Communications, Privacy, and Protected Health Information

(a) Contact form; do not send sensitive health information. Our contact form asks for your first and last name, email, phone number, type of inquiry, and a message. Please keep your message general. Do not include diagnoses, medications, treatment or substance-use history, Social Security numbers, insurance member IDs, or payment card data. Web forms, email, and text messages are not guaranteed to be secure. If you choose to include such information, you do so at your own risk. We use what you submit to respond to your inquiry and for the purposes in our Privacy Policy.

(b) HIPAA and substance-use-disorder confidentiality. Where we act as a covered entity or a part 2 program, information we create or receive in providing treatment is protected under HIPAA (45 C.F.R. Parts 160 and 164), 42 C.F.R. Part 2, the California Confidentiality of Medical Information Act, and other applicable law, and is handled as described in our Notice of Privacy Practices. Information you submit through the Site before becoming a patient may not be protected health information. Nothing in these Terms limits any right you have under those laws.

(c) No guaranteed response. Submitting the contact form does not mean we have received, reviewed, or accepted your inquiry, and we do not guarantee a response by any particular time. Do not use the contact form in an emergency (see Section 2).

(d) Call monitoring. Calls with our team may be recorded or monitored for quality, training, and compliance. By continuing after notice, you consent to this. If you do not consent, stop the communication and contact us by another method.

(e) Calls and texts. By providing your phone number and submitting our contact form, you expressly consent to receive calls and text messages (including by automated technology) from us regarding your inquiry, admissions, and scheduling. Consent is not a condition of purchase or treatment. Message and data rates may apply. Reply STOP to opt out of texts at any time.

(f) Privacy Policy. Our Privacy Policy at www.blaisehealth.com/privacy-policy is incorporated into these Terms.

5. Accuracy of Information

We try to keep the Site accurate but do not warrant that its content is complete, current, or error-free. Content may contain typographical errors, outdated information, or omissions, including regarding services, pricing, availability, and licensing. We may correct or change content at any time without notice and have no duty to update it.

6. Intellectual Property

(a) Our property. The Site and all content, including text, graphics, logos, photographs, videos, audio, software, designs, databases, and the selection and arrangement of them (the "Content"), and our names, logos, and marks (the "Marks") are owned by or licensed to us and protected by copyright, trademark, and other laws. All rights not expressly granted are reserved.

(b) Limited license. Subject to these Terms, we grant you a limited, revocable, non-exclusive, non-transferable, non-sublicensable license to access the Site and view and print Content for your personal, non-commercial use.

No other use is permitted without our prior written consent, including copying, republishing, framing, scraping, selling, or creating derivative works.

(c) Breach. Any violation of this Section is a material breach and automatically terminates your license.

7. Your Submissions and Feedback

(a) "Submissions" means any inquiry, message, suggestion, feedback, or other information you send us through the contact form or otherwise through the Site, other than protected health information handled under a treatment relationship.

You grant us a perpetual, irrevocable, worldwide, royalty-free, fully paid, transferable, sublicensable license to use, reproduce, modify, publish, and distribute Submissions for any lawful purpose, without credit or compensation, and, to the extent permitted by law, you waive moral rights in them.

(b) You represent that you own or have the rights to your Submissions, that they are accurate and lawful, that they do not infringe any person's rights, and that they do not disclose another person's confidential or health information without authorization.

(c) We will not use your name, image, or testimonial in marketing without your separate written authorization. Nothing in this Section authorizes use or disclosure of protected health information in violation of law.

(d) We may, but have no obligation to, monitor, edit, refuse, or remove any Submission.

8. Prohibited Conduct

You agree not to:

  • use the Site for any unlawful, fraudulent, or unauthorized purpose, or in violation of any law or regulation;

  • submit false, misleading, or another person's information, including false insurance information, or impersonate any person;

  • use the Site to solicit, broker, or refer patients, or to receive or offer any payment or remuneration for referrals, in violation of any federal or California law (including patient-brokering, anti-kickback, and anti-solicitation laws);

  • scrape, crawl, harvest, or systematically retrieve data or content, or use bots, spiders, or automated tools to access the Site (other than standard search engines);

  • reverse engineer, decompile, or copy the Site's code, or circumvent, disable, or interfere with security, access, or copy-protection features;

  • upload viruses, malware, tracking pixels, or other harmful or surveillance material, or overload or disrupt the Site or its networks;

  • harass, threaten, or abuse our staff, patients, or other users, or use information from the Site to harass or harm anyone;

  • post or send content that is defamatory, obscene, hateful, discriminatory, or infringing;

  • use the Site to compete with us, advertise, or sell goods or services, or for any commercial purpose without our written consent;

  • frame or link to the Site in a misleading way or in a manner that implies affiliation or endorsement;

  • delete any copyright or proprietary notice;

  • make false reports or improper use of our contact or support channels; or

  • assist or encourage anyone to do any of the above.

Nothing in this Section restricts truthful reviews or statements you are legally entitled to make, or any right that cannot be waived under law.

9. Third-Party Sites, Content, and Providers

The Site may link to or display third-party websites, directories, labs, payment processors, insurers, and other content ("Third-Party Content"). We do not control, endorse, or assume responsibility for Third-Party Content, including its accuracy, safety, or privacy practices. Your use of it is at your own risk and governed by the third party's terms. Any third-party providers, referral partners, or aftercare services are independent and are not our agents or employees, and we are not responsible for their acts or omissions.

10. Electronic Communications and Signatures

You consent to receive notices and records from us electronically and agree that they satisfy any legal requirement that communications be in writing. You agree that electronic signatures, clicks, and checkbox acceptances have the same legal effect as handwritten signatures, to the fullest extent permitted by law (including the federal E-SIGN Act and California's Uniform Electronic Transactions Act). This Section does not apply to documents that law requires to be in non-electronic form.

11. Changes to the Site and the Terms; Availability

We may modify, suspend, or discontinue any part of the Site at any time without liability. We may revise these Terms at any time by posting the updated version with a new "Last updated" date. Your continued use of the Site after posting constitutes acceptance. For material changes to Section 17 (Dispute Resolution), the changes apply only to disputes arising after the effective date. We do not guarantee that the Site will be uninterrupted, secure, or error-free.

12. Suspension and Termination

We may suspend, restrict, or terminate your access to the Site at any time, with or without notice, for any lawful reason, including breach of these Terms. Sections that by their nature should survive (including Sections 4, 6, 7, 13 through 17, 19, and 20) survive termination. Termination of Site access does not affect your treatment, which is governed by your admission agreement and applicable law.

13. Disclaimer of Warranties

THE SITE AND ALL CONTENT ARE PROVIDED "AS IS" AND "AS AVAILABLE." TO THE FULLEST EXTENT PERMITTED BY LAW, WE DISCLAIM ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, ACCURACY, TITLE, AND NON-INFRINGEMENT. We do not warrant that the Site or Content is accurate, complete, current, secure, virus-free, or uninterrupted, or that using it will achieve any result. We make no warranty regarding any third-party product, service, or website. This Section does not limit any clinical standard of care that applies to treatment you receive from us under a separate agreement.

14. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY LAW, (A) WE AND OUR OWNERS, DIRECTORS, OFFICERS, EMPLOYEES, CLINICIANS, CONTRACTORS, AFFILIATES, AND AGENTS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR FOR LOST PROFITS, REVENUE, DATA, OR GOODWILL, ARISING OUT OF OR RELATED TO THE SITE OR THESE TERMS, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES; AND (B) OUR TOTAL AGGREGATE LIABILITY ARISING OUT OF OR RELATED TO THE SITE OR THESE TERMS, UNDER ANY THEORY, WILL NOT EXCEED THE GREATER OF $100 OR THE AMOUNT YOU PAID US, IF ANY, FOR USE OF THE SITE IN THE 12 MONTHS BEFORE THE CLAIM AROSE.

These limitations apply to claims arising from your use of the Site, reliance on Site content, interaction with the contact form, and third-party content or conduct. They do not limit liability that cannot be limited under applicable law, including liability for fraud, willful misconduct, or violation of law (California Civil Code § 1668), and they do not apply to professional malpractice or negligence claims arising from clinical treatment you receive under a separate admission agreement. Some jurisdictions do not allow certain limitations, so some of the above may not apply to you. If so, our liability is limited to the maximum extent permitted by law.

15. Indemnification

You agree to defend, indemnify, and hold harmless the Company and its owners, officers, directors, employees, clinicians, contractors, affiliates, and agents from any claim, loss, liability, damage, cost, or expense (including reasonable attorneys' fees) arising out of: (a) your use of the Site; (b) your breach of these Terms or your representations; (c) your Submissions; (d) your violation of law or of any third party's rights, including intellectual property, privacy, or publicity rights; or (e) any harmful or unlawful act by you toward another person. We may assume exclusive defense and control of any matter subject to indemnification at your expense, and you will cooperate. You may not settle any such claim without our prior written consent.

16. Governing Law

These Terms and any dispute arising out of or relating to them or the Site are governed by the laws of the State of California, without regard to conflict-of-law principles, and, as to Section 17, by the Federal Arbitration Act (9 U.S.C. § 1 et seq.). The U.N. Convention on Contracts for the International Sale of Goods and the Uniform Computer Information Transactions Act do not apply.

17. Disputes

(a) Contact us first. If you have a concern about the Site, please contact us at contact@blaisehealth.com and we will try to resolve it informally. Before filing a lawsuit, please send us written notice describing the issue and allow at least 30 days for us to respond.

(b) Courts. Any dispute arising out of or relating to these Terms or the Site will be brought in the state or federal courts located in Los Angeles County, California, and you and we consent to the jurisdiction of those courts. Either party may instead use small-claims court.

(c) Treatment. Disputes about treatment you receive from us are governed by your signed admission agreement, not by this Section.

18. California Residents and Other Notices

California residents have the privacy rights described in our Privacy Policy. Nothing in these Terms waives rights that cannot be waived under California law.

Accessibility. We are committed to making the Site accessible. If you have difficulty using it, contact us at contact@blaisehealth.com and we will work to provide the information in an accessible format.

19. Copyright Complaints (DMCA)

If you believe content on the Site infringes your copyright, send a notice that complies with 17 U.S.C. § 512(c)(3) to: contact@blaisehealth.com. We may terminate access of repeat infringers.

20. General Provisions

(a) Entire agreement. These Terms, our Privacy Policy, and any other policies posted on the Site are the entire agreement between you and us about the Site and supersede prior understandings regarding the Site.

(b) Severability. If any provision is held invalid or unenforceable, it will be enforced to the maximum extent permissible and the remaining provisions remain in full force, subject to Section 17(k).

(c) No waiver. Our failure to enforce any right or provision is not a waiver of it.
(d) Assignment. You may not assign these Terms without our consent. We may assign them freely, including in a merger, acquisition, or sale of assets.
(e) Force majeure. We are not liable for any failure or delay caused by events beyond our reasonable control.

(f) No agency. No partnership, joint venture, employment, or agency relationship is created by these Terms or your use of the Site.

(g) No construction against drafter. These Terms will not be construed against us as drafter.
(h) Electronic form. You waive any defense based on the electronic form of these Terms or the absence of a physical signature.

(i) Headings and interpretation. Headings are for convenience only. "Including" means "including without limitation."

(j) Eligibility. You must be at least 18 years old to use the Site. A parent or legal guardian may use the Site on behalf of a minor and accepts these Terms on the minor's behalf.
